2024 NYC SCHOOL SURVEY: FAMILY SURVEY

PS82 invites all families to complete this year’s annual school survey. Completing the annual NYC School survey is one of the best ways to inform school leaders, educators, and families about what’s working and what’s not working in your child’s school. Please TAKE THE TIME TO COMPLETE THE NYC SCHOOL SURVEY!

Family Survey Access

Families can choose to take the survey online or on paper:

Each paper survey will have the student’s name printed at the top, along with one of the prepaid envelopes provided.

You can also take the survey online at :

NYC SCHOOL SURVEY 2024

(in any of the nine NYCDOE supported languages) using their survey access code, which is also printed on the survey. The family access code is “f” followed by the child’s nine-digit Student identification (OSIS) number. Note that “f” must be lower case. Families can find their child’s Student Identification (OSIS) number on their report card, student ID card, or NYCSA account.

Instructions for paper surveys

  • Use black ink, blue ink, or pencil for bubbling in responses.
  • Fill in the selected answer bubbles completely.
  • Do not include any stray marks on the page.
  • Do not write over the QR code.
  • Do not tear the paper or attempt to remove the staple.
  • See image below for proper bubbling in.

ENCUESTA ESCOLAR DE NUEVA YORK 2024: ENCUESTA FAMILIAR

PS82 invita a todas las familias a completar la encuesta escolar anual de este año. Completar la encuesta anual de NYC School es una de las mejores maneras de informar a los líderes escolares, educadores y familias sobre lo que funciona y lo que no funciona en la escuela de su hijo. ¡POR FAVOR, TÓMESE EL TIEMPO PARA COMPLETAR LA ENCUESTA ESCOLAR DE NUEVA YORK!

Acceso a encuestas familiares

Las familias pueden optar por realizar la encuesta en línea o en papel: Cada encuesta en papel tendrá el nombre del estudiante, junto con uno de los sobres prepagados proporcionados.

También puede realizar la encuesta en línea en www.nycschoolsurvey.org (en cualquiera de los nueve idiomas admitidos por NYCDOE) utilizando su código de acceso a la encuesta, que también está impreso en la encuesta. El código de acceso familiar es "f" seguido del número de identificación de estudiante (OSIS) de nueve dígitos del niño. Tenga en cuenta que "f" debe estar en minúsculas.
